WHAT IS CDS?

Consumer Directed Services (CDS), also known as Self Directed Care services, are home healthcare services offered by Medicaid waiver programs where YOU (the Consumer/Patient) can hire the caregiver of your choice and coordinate the authorized services they provide.

Consumer Directed Services (CDS) empower you to manage your own care with professional support. This flexible approach allows you to make decisions about the services you receive, ensuring they align with your personal preferences and needs. Our CDS program provides the autonomy and control you desire while offering the expertise and assistance necessary to enhance your quality of life.

BENEFITS OF CDS

There are many benefits to using CDS to direct your healthcare services:

• Comfort– CDS enables you to live independently in the comfort of your own home instead of receiving services you need in a nursing home or other
• Trust – Your ability to choose a family member or a friend to provide the services you need assures your trust in the care you receive.
• Flexibility – Hiring the caregiver of your choice gives you flexibility in scheduling the caregiver to meet your personal preferences.
• Support – By hiring the caregiver of your choice, you may be able to support a family member or friend who may need additional income.

CARE OPTIONS / SERVICES

CDS offers Personal Care, Respite Care, and/or Companion Care services. It DOES NOT include nursing services or services provided to other individuals in the household. Participants my quality for some or all of these services, which include, but are not limited to:

• Personal Care – bathing, grooming, dressing, personal hygiene.
• Toileting – ostomy or catheter hygiene, bowel and/or bladder routine, general toileting activities.
• Health – use of transfer devices/mobility issues/prostheses, passive range of motion, manual assistance with medications, and treatments, cleaning and maintenance of equipment.
• Housekeeping– cleaning, dusting, bed linens, laundry, trash.
• Meals– meal preparation and/or assistance with eating, washing dishes.
• Transportation– essential shopping/errands, school or employment, paying bills/banking, picking up medication.

Additional services can be provided through Medicaid Waiver programs. Contact us for details and, if you qualify, to initiate your CDS services.

QUALIFICATIONS

To participate in the CDS program, the Patient must meet the following qualifications:

• Be eligible for Medicaid in Missouri.
• Be physically disabled.
• Be 18 years of age or older.
• Be able to direct his or her own care.
• Be capable of living independently with the services of a personal care attendant.
• Require at least a nursing facility level of care.
