Hiring a personal caregiver for your loved one is a very important decision for families. Here at Breakthrough Healthcare, we understand the importance of choosing a caregiver who fits your family’s needs, personality, and lifestyle. That’s why we’ve expanded our caregiver services to include CDS (Consumer Direct Services) giving you the ability to choose your own personal private caregiver.
CDS is a Medicaid approved service that allows individuals to live at home while being taken care of by someone they choose. This can be any trusted person (other than a spouse) chosen by the patient or family.
Breakthrough Healthcare is an approved CDS provider for the state of Missouri. We serve St. Louis City & County, St. Charles, Franklin, and Jefferson Counties. Through this state-funded Medicaid program for elderly and disabled adults, we offer you the ability to choose your own personal caregiver who is paid through Medicaid. If you have a family member who you want to provide personal care services to your loved one, through CDS, they can be paid for their service – at no expense to your family. If you know someone, anyone you trust and approve, who you would like to provide personal care services for your loved one, through CDS, they can be paid for their service – at no expense to your family.

Maintain your independence and keep living at home with a little bit of help from us. If you don’t have a personal caregiver, we will assist you with finding a qualified caregiver with experience, compassion, and joy for the work they do. Our caregivers take pride in their work. CDS consists of personal care (bathing, cleaning, and meal preparation). Your personal and homecare needs will be met.
Consumer-directed services (CDS), although similar in nature to in-home services, are provided to persons with a physical disability and do not include any task performed by a licensed professional. A participant of CDS must be able to direct the care planning process and hire an attendant.
To qualify to receive CDS, you must…
- Be eligible for MO HealthNet (Medicaid).
- Be 18 years of age or older.
- Be able to direct his or her own care.
- Be physically disabled.
- Be capable of living independently with the services of a personal care attendant.
- Require at least a nursing facility level of care.
